Documentation Criteria
for Faculty/Staff & Employees

The Office of Disability Services (ODS) provides reasonable accommodations for eligible Emory University Faculty/Staff and Emory Healthcare employees with temporary or chronic medical conditions and/or disabilities.

To ensure that our procedures are fair and equitable, an accommodation or access (in most cases) will not be considered or granted unless the appropriate documentation is submitted to the Office of Disability Services. In addition, you must meet the eligibility criteria and follow the registration process in its entirety.


Please Note:

  • When requesting an employee accommodation, the employee must provide ODS with valid and complete documentation which substantiates the individual’s chronic medical condition and/or disability.
  • Documentation will state the nature and impact of the medical condition and/or disability and will be used to assist ODS staff in establishing what reasonable accommodation is warranted.
  • ODS will work cooperatively with department/unit supervisor(s) to ensure that adjustments allow the employee to meet the essential functions of his/her job assignment.
  • All documentation must be current (medical statements must fall within the calendar year for which accommodations and/or related services have been requested). Signature of the qualified healthcare professional(s) must be included.

Requirements to Submit Documentation

  1. A typewritten statement from a qualified health care provider on official letterhead substantiating the medical condition with a verifiable signature;
  2. Specific diagnosis(es);
  3. If applicable, current prescriptions and their effects (i.e. benefits, side effects, etc.) on the employees ’s ability to meet the expectations of the job assignment;
  4. Summary of the effects of the medical condition and/or disability on the employee’s ability to meet the expectations of the job assignment;;
  5. Detailed recommendation of suggested accommodation(s). Please specify limitations and/or restrictions, include time frames (if applicable) and indicate whether the medical condition and/or disability is temporary or permanent.
  6. Submit documentation and correspondence by mail or fax:

When ODS has received the documentation, the employee will be contacted by a member of the ODS staff to complete the registration process.


ODS maintains confidentiality, (particularly the diagnosis(es) as appropriate. However, in some instances the ODS may be required to disclose (employee’s permission granted in advanced) information that is pertinent to the successful negotiation of reasonable access and workplace accommodations.
