Forms for Students
The Office of Disability Services (ODS) assists eligible students with temporary or chronic medical conditions and/or disabilities in obtaining a variety of services and ensures that all matters of equal access, reasonable accommodation, and compliance are properly addressed.
To ensure that our procedures are fair and equitable, an accommodation (in most cases) will not be considered or granted unless you submit the appropriate and completed forms to ODS. Alternate formats are available upon request.

When ODS has received the form(s), the student will be notified.
