Documentation, Forms, & Accommodations for Students

Submitting Documentation Criteria to ODS

Appropriate documentation is required to determine the current nature and impact of a disability/medical condition and develop a comprehensive, reasonable Individualized Accommodation Plan (IAP). All students are required to self-ID and complete the applicable components in the registration process as set forth by the Office of Disability Services. An accommodation will not be considered or granted unless the appropriate documentation is submitted as outlined in the ODS Documentation Criteria, which is based on the "Georgia Board of Regents Criteria for Evaluation of Students with Learning Disabilities."

Please note: Any documentation that exceeds three years from the time of the request will not be acceptable. All information received will remain confidential and will not be included in the student's academic file. Failure to follow the outlined instructions below when submitting documentation to the Office of Disability Services (ODS) may delay or impede our efforts to complete this part of the process in a timely manner.
  1. A type written statement from a certified health care provider on official letterhead, including a specific diagnosis. Any documentation received without a verifiable signature Is not valid.
     
  2. If applicable, please indicate any current prescriptions and the effects of each medication (i.e. benefits, side effects, etc.) on the student's ability to function within the academic environment.
     
  3. Summary /Recommendation(s) of how the disability and or medical condition currently affects the student's environment (i.e. suggested accommodation(s). Please be certain to detail what you believe will be required for full access and participation in all aspects of the academic environment as relevant to the diagnosis and test results.
     
  4. Documentation may be received via fax at 404-727-1126 or mailed to:
    ODS at Emory University,110 Administration Bldg., Atlanta GA 30322.
     
  5. Once ODS has confirmed the receipt of the documentation, the student will be notified to schedule an intake appointment with the Plan Developer.
     
  6. An Individualized Service Plan (IAP) will be developed for each student dependent on his or her needs.
