Documentation Criteria for Students

The Office of Disability Services (ODS) provides reasonable accommodations for eligible students with temporary or chronic medical conditions and/or disabilities.

Please Note:

  • For all accommodations, appropriate documentation is necessary as outlined in this documentation criteria for students. In addition, you must follow the registration process for students for consideration for any accommodations.
  • When requesting an educational accommodation, it is the student’s responsibility to provide ODS with valid and complete documentation which verifies the individual’s chronic medical condition and/or disability. Documentation will state the nature and impact of the medical condition and/or disability and will be used to develop a comprehensive, reasonable Individualized Accommodation Plan (IAP) for the student.
  • All documentation which exceeds three years from the time of the request will not be accepted.

Requirements to Submit Documentation

  1. A typewritten statement from a certified health care provider on official letterhead, including a specific diagnosis. Statement must include a specific diagnosis AND a verifiable signature;
  2. If applicable, healthcare provider(s) will indicate current prescriptions and their effects (i.e. benefits, side effects, etc.) on the student’s ability to function within an academic environment;
  3. Healthcare provider(s) will submit:
    1. A summary of the effects of the students medical condition and/or disability; AND
    2. A detailed recommendation of the accommodations that will provide the student with equal access to Emory’s programs and services.
  4. Students who require accommodations on the basis of an ADD/HD diagnosis must also meet the criteria for Learning Disability (LD). Refer to Attention Deficit Disorder/Hyperactivity Disorder(ADD/HD) Criteria and Learning Disability (LD) Criteria.
  5. Submit documentation and correspondence by mail or fax:

When ODS has received the documentation, the student will be notified and an intake appointment with a member of the ODS staff will be scheduled.
